Registration:
Persons who want to get vaccinated have to click on the link in the description below. This will lead them to the registration page. They have to fill out the following details:
Name
Email (must be real)
Phone Number
Date of Birth
Insurance Number (must be 10 digits)
Sex (Gender)
Full Address

Additionally, they are asked to declare risk group related information by clicking one or more of these check boxes:
Allergies
Heart Disease
Other

Regarding the profession, they are asked, if it applies, to check one or both of these check boxes:
Health
Educational

Finally they have to check the box to agree to our data protection statement (mandatory) and click the submit button.

Vaccination Date:
Vaccinees are then taken to a new page automatically, where they can choose one of three dates(vaccination slots) to get vaccinated.
Notice: The time is the real time, but the date is a date in the future for simulation purpose. You will always be vaccinated on same date of registration at the time of your vaccination confirmation. If you registered past a certain time, the vaccination will be done on the next day at this time.

Confirmation:
After choosing one the process for the vaccinees is over. They will receive an Email, confirming their date and time of the vaccination.

Survey:
After their vaccination date and time (same day or next) the final step for the vaccinees takes place. After the doctors simulated the vaccination, the vaccinees will receive an Email, confirming that they just got vaccinated.
This email includes a link to a survey, which the vaccinee is asked to participate in.

The questionnaire will ask you for the following information:
Gender
Age
Chronic Illnesses
Allergic Reaction

Furthermore they will be asked about symptoms they experienced after the vaccination by checking one or more of the check boxes representing Fever, Soreness, etc.

Finally they are asked to express their sentiment by choosing a raging between 1 and 5 about
Intuitivity of the scheduling system
Waiting Time (real vaccination time minus scheduled time slot)
Satisfaction with the overall vaccination process
After clicking the submit button, the process is over for the vaccinees.

Please, do not eneter 111111.., 222222.., 1234567890, etc. for Insurance Number. Please choose a 10 digits random number.

I tested the application with a random insurance number. First, it took really long to load the application (around 40 seconds) and I had to reload the page once, because of an error. The frontend worked well, but once I completed the task, I did not get any email, even not after 15 minutes of waiting.

The application seems to work generally, but it took very long to load, but the front end was fine. I got 5 emails though, which was a bit confusing and certainly not intented.

We looked at our data and realized where the bug is. The app from now will not send more than one email. Thank you for your feedback.

I got the email within one minute, so I guess the workflow works fine, in my opinion the interface could be a little bit cleaner

Thanks for the feedback. Sadly we know that the server is not really fast and sometimes it even shows you that the server is not up. That is because of Heruko, if no one is "online" in the application they shut down the servers and they need to be started again. (Semms to be "normal" if you do not pay for a service) We are trying to go to the application on our own from time to time to avoid it but we do not have the time to always be online :(. Regarding the email - it really can take a while. In our testing phase we discovered that some of us got the mail in the second they clicked and others (like me) waited more than 24 hours. We are not sure what the underlying problem of that is. It could be also Heruko or the providers itself that they block the messages. But in the end we all received our mails.

We use free services through the app, so there can be issues in performance or time it takes to receive the email, but it should come in 15 min. Also, sometimes it can go to your spam folder so please check them also. (as stated on the app)

The application works. But I have not received any mail although I tried it on three different days.

The UI is nicely designed and the registration process as well as the date selection worked without any problems. I am still waiting for the email.

Worked as described. I used gmail and got the email.

The application works very well and as intended, so I have almost nothing to criticise. One small detail is that the questionnaire should use radio buttons instead of checkboxes for the answers to the question about the age of the patient, so that only one answer can be selected. But this is only a tiny detail, otherwise the application worked very satisfactorily.

Works fine ! :) (survey mail will come tomorrow 10am) I just entered 26.06.2021 as my birthday (which obviously isn't possible)

Everything worked quite fine. I also received a mail for both tests I did. (There is a typo in the date of time slot 3)

The hints that the website often does not work right away was good -> it was the same for me that the page did not load. I tested the application with two different mails - with GMAIL I really got the mail within seconds! So it worked very well for me.

Initially I never received an Email and I thought the app was broken, but after coming back to this page I read the comments and retried it with Gmail - now I also got the Email and could complete the workflow. Works perfectly, in a production scenario the mailing should of course work for all providers but for Demo purposes this works just fine. Well done!
